Name - Bhavya Ranjan Roll - 17  Batch - 2019  A 50 year old housemaker, who is a resident of Nalgonda came to OPD with chief complaints of- Heavy and prolonged menstrual bleeding since 1 month. History of presenting illness   Patient was apparently asymptomatic 1 month ago , then she developed 1 month amenorrhea, followed by heavy and prolonged cycle, continuous bleeding for 11 days, associated with clots of size 5×5cm. Bleeding subsided on medication. 5 days back she passed a clot of size 3×3 cm two times. H/o generalized weakness since 1 month. H/o weight loss since 1 year. H/o loss of appetite since 1 month. No h/o abdominal distension, post costal bleeding, fever , cough and cold. History of past illness K/c/o HTN since 20 years  K/c/o DM since 2 years  No TB, Asthma, CAD, Seizers. Name - Bhavya Ranjan  Roll - 17  Batch - 2019 A 60 years old male patient, resident of Nakrekal who is farmer by occupation came to opd with chief complaints of- - tightness of abdomen since 1 week  - shortness of breath since 1 week -  decreased urine output since 15 days  HISTORY OF PRESENTING ILLNESS  Patient was apparently asymptomatic 15 days ago when he developed high grade fever which was associated with chills and rigor. A/w decreased urine output and buring micturition with hesitancy. Patient also complaints of blotting in abdomen which is not associated with food intake, belching and regurgitation but a/w shortness of breath.  He developed Anasarca. B/l pedal edema present. Not a/w pain in abdomen, nausea, vomiting and loose stool. Not a/w chest pain, palpitations and cough.  51 year old male patient who is resident of chityal ,and works in a transportation company came to the hospital with complaints of  1- Fever since 10 days 2- Cough since 10 days  3-shortness of breath since 6 days  History of presenting illness :  Patient is apparently asymptomatic 10 days back then he developed. Fever since 10 days which is high grade , with chills and rigors , intermittent ,relieving with medication. Associated with cough and shortness of breath. Cough since 10 days which is productive ,mucoid in consistency,whitish ,scanty amount ,more during night times and on supine position ,non foulsmelling ,non bloodstained . Right sided chest pain - diffuse , intermittent ,dragging type , aggravated on cough ,non radiating ,not associated with sweating , palpitations. Shortness of breath since 6 days , insidious onset , gradually progresive ,of grade 3 - (MMRC scale ),not associated with wheeze ,no orthopnea ,no Paroxysmal nocturnal dyspnea, no pedal edema .  History of pain
